Management of DOAC dosages in case of renal failure

In the case of renal insufficiency (Chronic Kidney Failure), the dosage of the DOAC drugs must be adapted according to the creatinine clearance (CrCl). The dosing schedule of the different DOACs according to the CrCl levels is illustrated in the EHRA 2018 guidelines:
• at levels of CrCl <15 ml/min, no DOAC can be used

• at CRCL levels 15-30 ml/min, Apixaban, Endoxaban Rivaroxaban can be used at a reduced dosage, after careful monitoring of the CrCl values (quarterly)
